I authorize Bohren’s Moving & Storage, Inc. to charge my Visa/Master Card, Amex or Discover Card for transportation and or other related charges. 

I acknowledge that, in reliance upon my execution of this credit card form, Bohren’s  will give up possession of my household, electronics, or special commodities goods to me without requiring payment in cash or its equivalent. 

 In the event the issuer of my credit cards cancels the charge or otherwise fails to pay Bohren’s the amount due, I shall be liable to Bohren’s for the amount due as well as all reasonable costs and expenses (including attorney fees) incurred in collecting the amount due and interest on all unpaid amounts at the rate of 11/2% per month. 

My liability to Bohren’s in this event must be paid by cash, certified check or money order.
